NDA/NDCL 2009 Athletic Hall of Fame Nomination Form

Nominees must have either participated in any of NDA or NDCL’s major interscholastic competitive sports as a player, coach, administrator, or in some other capacity which displays his or her interest and loyalty to NDA and/or NDCL. Candidates should be recognized as having had exceptional skill and/or dedication to their sport(s). Recommendations should include a summary of your candidate’s athletic achievements while at NDA or NDCL. If your nominee has used his or her skills to enrich the community by volunteering as a coach and/or teacher, please be sure to include that information. Nominees must have graduated in 2005 or earlier. For complete eligibility requirements, see the “Hall of Fame Selection Guidelines”.

ALL NOMINATIONS MUST BE RECEIVED BY THE SELECTION COMMITTEE BY APRIL 1, 2009.
